Insurance Information
Our office will make every effort to provide you with the finest care and the most convenient financial options. To accomplish this, we work with you to maximize your insurance reimbursement for covered procedures. Before your first visit, please verify your dental coverage. Please bring your insurance plan information with you on your first visit. We will work with you to ensure that you receive the maximum benefits to which you are entitled. Most importantly, please keep us informed of any insurance changes such as policy name, insurance company address, or a change of employment.

Our Office Policy Regarding Dental Insurance
If your child is covered by your dental insurance plan, please familiarize yourself with your individual policy. We submit dental insurance as a courtesy for our patients. We do not have a contract with your insurance company, only you do. We are not responsible for how your insurance company handles its claims or for what benefits they pay on a claim. We can only assist you in estimating your portion of the cost of treatment. We at no time guarantee what your insurance will or will not do with each claim. We also cannot be responsible for any errors in filing your insurance.
We require any deductibles, co-payments, and estimated patient portions be paid at the time of service. You, not your insurance, are ultimately responsible for payment of services rendered in our office. After dental work has been performed in our office, our staff will bill your insurance company as a courtesy to you, based on the insurance information you have provided. We are only able to estimate your financial portion. If your insurance pays differently than estimated you are responsible for the unpaid balance. If for any reason your insurance does not pay, you are personally responsible for payment of all services.
By law, your insurance company is required to pay each claim within 30 days of receipt. We file all insurance electronically so your insurance company will receive each claim within days of the treatment. You are responsible for any balance on your account after 30 days, whether insurance has paid or not. Unpaid balances over 60 days will accrue a monthly fee equal to 18% APR. Balances over 90 days will be turned over to a collection agency; in this event, you will be responsible for all collection and legal fees.
